Excalibur's Telemedicine Credentialing

EXCALIBUR HEALTHCARE CREDENTIALING POLICY STATEMENT

It is the policy of Excalibur Healthcare to ensure that licensed independent practitioners meet Joint Commission requirements for credentials, privileging and performance standards. Credentialing is performed jointly with all physicians prior to appointment to Excalibur Medical Staff. The providers attest that all information submitted for credentialing and privileging process is accurate and agree to immediately report any changes in status of the information maintained in the Credentialing files.

 

If any submitted items differ from documentation disclosed through the verification process, the Credentialing Manager will consult with the provider to resolve the discrepancies. All time sensitive documents for any applicant or re-applicant must be no more than 90 days from the date of the provider’s attestation at the time of the credentialing review. In the event time sensitive documents are found to be out of compliance with regulatory guide, those documents will be re-verified prior to review by the Credentialing manager.

 

CMS allows the governing body of a hospital or CAH (Critical Access hospitals) whose patients are receiving telemedicine services to grant privileges in reliance on the credentialing and privileging decisions made by the distant-site hospital. This also permits hospitals and CAHs to rely on credentialing and privileging decisions made by “distant-site telemedicine entities,” such as teleradiology groups and ambulatory surgery centers. Excalibur Healthcare is Joint Commission accredited and strives to provide reasonable assurance that our doctors meet or exceed hospital or CAH requirements.

 

All applications for clinical privileges and reappointment of privileges will be evaluated based on current licensure, education, training or experience, current competence and ability to perform the clinical privileges requested. All Medical Staff credentials will be re-verified every two years.
